- 3 pcs. Latest 2×2 picture
- Latest Barangay Clearance
- Latest Proof of Income
- If business – Business Permits, Picture of Business, Latest FS, Proof of Business Transaction
- Additional Requirements:
- If corporation – General Information Sheet (GIS), Articles of Incorporation and By-laws
- If partnership – Lists of Partners, Articles of Partnership/Contract of Partnership
- If employed – Original Certificate of Employment, 3 Months Latest Payslip
- If business – Business Permits, Picture of Business, Latest FS, Proof of Business Transaction
- Proof of Billing
